Qualifications

• Bachelor’s degree or 2+ years of experience in the field.
• Strong communication in English, both written and verbal.
• Experience coordinating healthcare or behavioral health services preferred.

Responsibilities

• Coordinate MOUD, behavioral health, and general health services.
• Serve as liaison between departments and community groups.
• Assist in discharge planning and prevent unnecessary hospitalizations.
• Maintain confidentiality and HIPAA compliance.
• Coordinate follow-up appointments and transportation.
