Basic Functions

To provide coordination of client-centered services that link clients with substance use disorder, behavioral health, and general health with coordinated care and psychosocial services. Working closely and collaboratively with the HUH inpatient and Consultation‑Liaison and Addiction Medicine Consult services and the ambulatory Mental Health Clinic (Behavioral Health Clinic) teams, the Care Coordinator assesses clients’ needs and support systems; facilitates timely, coordinated access to and continuation of medically appropriate substance use disorder, behavioral health, and general health care health and support services; assists in expediting appropriate discharge from inpatient facilities, as medically appropriate, and assists in preventing unnecessary hospitalizations; and establishes and maintains relations with case managers, healthcare providers, social services and other agency representatives to bridge the gaps between health, social service, entitlements and other systems.


Principal Accountabilities


  • Assists in development and implements protocols for coordinating the substance use disorder, behavioral health, and general health services for individual clients or patients with substance use or co‑occurring psychiatric and substance use disorders.

  • Provides care coordination services to a caseload of patients/clients, as assigned by the PI, Project Director, or Care Manager Coordinator.

  • Operates as a liaison between the Departments of Psychiatry and Community and Family Medicine.

  • Social Work and the DC Department of Health and other community groups.

  • Ensures that patients receive necessary follow‑up substance use or psychiatric disorder treatment and medical treatments.

  • May accompany patients to the initial or follow‑up visits and may help arrange transportation and scheduling of appointments.

  • Conducts intake and follow up assessments of clients’/patients’ behavioral health and medical/functional status; structural vulnerabilities; psychological; educational; financial; and individual support needs.

  • Reviews all initial client bio‑psychosocial assessments and reassessments.

  • Attends department trainings and meetings relevant in ongoing effort to increase knowledge and skills to work effectively with clients.

  • Attends annual and mandatory compliance Program training, including the Health Insurance Portability and Accountability Act (HIPPA) Privacy training, maintains adherence to the Health Sciences Compliance Program, the Howard University Code of Ethics and the Health Sciences Standards of Conduct.

  • Works on behalf of patients receiving medications for opioid use disorder treatment to improve the system of care by identifying service gaps and identifying services that are needed and ensuring that these services are provided in a courteous and dignified manner.

  • Maintains confidentiality of patients, their families and the staff.

  • Performs other related duties as assigned.


Core Competencies


  • Knowledge of the care needed for individuals receiving MOUD or other substance use disorder treatment.

  • Knowledge of community resources to establish referral systems with community agencies to facilitate coordinated management plans for patients and their families.

  • Ability to express ideas clearly using appropriate language in communicating with a wide range of health, socioeconomic, and cultural groups.

  • Competence in both oral and written English.

  • Ability to establish and maintain effective and harmonious work relations with clients and their family members and other treatment or service providers; physicians, staff, University and Hospital personnel, and the general public.

  • Proficiency in both oral and written English.


Minimum Requirements

A Bachelor’s degree in Social Work, Counseling, or a related field, or 2 or more years of experience in the field.
