Job summary

Advocate Health is seeking a Case Manager at Floyd Medical Center in Georgia. The Case Manager develops, implements, and evaluates outcomes for defined patient populations using the nursing process, and collaborates with a broad care-team to coordinate services.

Role emphasizes advocacy, continuity of care, and cost-effective outcomes; Monday–Friday collaboration with the case management team and on-call availability as needed. RN licensure in GA/AL is required, BSN preferred.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses; BSN preferred, MSN encouraged.
• Minimum of five years of experience in care of the defined patient population; case management experience encouraged.
• Current RN license to practice in Georgia or Alabama; case management certification encouraged.

Responsibilities

• Utilize nursing process to develop, implement, and evaluate case management outcomes.
• Collaborate with care team to ensure continuity and coordination of services.
• Educate, consult, and assist in management of identified patient population.
• Act as liaison among physicians, nurses, rehab, pharmacy, and other disciplines to achieve quality care and cost effectiveness.
