Job summary

The Case Manager at Floyd Medical Center (Admission Discharge Transition Unit) is a full-time role reporting to the Director of Coordinated Care. It applies the nursing process to develop, implement, and evaluate case management outcomes for defined patient populations and collaborates with a multidisciplinary care team.

Responsibilities include advocacy and coordination of services to optimize outcomes while controlling costs.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses; BSN preferred; MSN encouraged.
• Minimum of five years of experience in the care of assigned patient population.
• Current registered nurse license in Georgia or Alabama as determined by job location; Case Management certification encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
• Collaborate with physicians, nurses, and staff to coordinate care and advocate for patients.
• Educate, consult, and assist in patient management to achieve high quality outcomes at cost-effective levels.
• Coordinate daily with the case management team to ensure timely achievement of patient outcomes.
