Job summary

Optum in Massachusetts, Quincy, is seeking a role focused on coordinated, patient-centered care for at-risk individuals. You will facilitate care plans, monitor adherence, and serve as a key liaison between primary care, behavioral health, and patients or families.

The position involves assessing social factors affecting care, tracking appointments and medications, and documenting in the EMR. Office-based role with onsite one day per week; primary home sites include Quincy or nearby locations.

Qualifications

• High School Diploma or GED required.
• 1+ years of experience in a social services or behavioral health setting.
• 1+ years of experience with electronic medical records (EMR)

Responsibilities

• Facilitates care for at-risk patients and tracks adherence.
• Acts as liaison between primary care, behavioral health teams, and families.
• Assesses social factors affecting care and documents in EMR.
• Tracks appointments, labs, referrals, and medication compliance.
• Extracts and analyzes data; prepares clinician reports and presentations.
