Job summary

Kidney-Specialists-of-Southern-Nevad in Las Vegas, NV seeks an Assistant Care Navigator to link KSOSN with partner facilities and dialysis organizations, guiding patients through value-based care, starting dialysis, and transplant listing processes.

The ACN supports Care Navigators with admissions, patient activation, data management, and education referrals; strong MS Office and Excel skills, and English communication are essential.

Qualifications

• High School Diploma or equivalent required.
• Minimum three (3) years of clerical, administrative experience with extensive use of spreadsheets.
• Minimum one (1) year of related experience in a fast-paced healthcare environment.
• Intermediate knowledge of Microsoft Office applications, especially Excel.
• Spanish a plus.

Responsibilities

• Coordinate patient coordination and support for admissions, activation measures and health evaluations.
• Follow up with patients lost to follow-up and support access placements.
• Coordinate dialysis placements and RPM initiatives.
• Obtain Kidney Smart referrals from EMR and refer patients to KDE programs.
• Track patient engagement with education programs and document enrollment.
• Track and record key metrics and import patient records into the EMR.
