Kidney-Specialists-of-Southern-Nevad is seeking a Care Navigator to act as a liaison among KSOSN, partner facilities, dialysis organizations, transplant teams and surgeons. Qualifications

• High School Diploma or equivalent required.
• Minimum three years in a fast-paced healthcare environment with contract and network benefits familiarity.
• Excellent customer service, communication and organizational skills.
• Ability to read, write, speak and understand English fluently.

Responsibilities

• Serve as the primary contact for initial referrals and admissions to dialysis.
• Act as liaison with referral sources, centers, patients and discharge planners; gather medical information quickly for placement.
• Maintain dialysis census, dialysis reports and vascular access/transplant data.
• Schedule high-priority patients for ongoing health examinations.
• Assist with patient admission, transplant and vascular access processes; collect insurance and other data.
• Coordinate with Intake/Authorizations to establish appropriate patient placement; follow KSOSN procedures.
• Communicate promptly with centers and resolve issues impacting placement; escalate denials as needed.
• Build professional relationships with referral sources; provide outstanding customer service.
