Job summary

Medasource is seeking an experienced Care Navigator to support patients and families in a fast-paced Emergency Department and Inpatient healthcare environment. You will engage directly with patients, identify needs, and connect them with healthcare, social, and community resources.

Ideal candidates have 2–5 years in ED or inpatient settings, strong communication, and the ability to coordinate with nurses, social workers, and other clinicians while maintaining patient confidentiality.

Qualifications

  • 2–5 years of relevant professional experience in ED or inpatient settings.
  • Strong verbal communication and interpersonal skills.
  • Ability to work independently while collaborating with clinicians.
  • Experience navigating healthcare resources and patient confidentiality.
  • Willingness to work Saturdays and Sundays 9:30 AM–6:00 PM.

Responsibilities

  • Engage directly with patients and families in Emergency Department and Inpatient settings.
  • Independently introduce yourself to patients, identify needs, and provide appropriate support.
  • Connect patients and families with available healthcare, social, and community resources.
  • Collaborate with nurses, social workers, and other clinical team members to coordinate services.
  • Help patients navigate available programs, resources, and next steps in their care.
  • Maintain professional boundaries while providing compassionate, patient-centered support.
  • Document patient interactions accurately and thoroughly.
  • Escalate clinical, social, or safety concerns to the appropriate member of the care team.
  • Remain calm and professional when supporting patients and families in stressful situations.

Job description

Schedule: Candidates will work 4 shifts per week, including:

  • 2 weekday shifts: either 8:00 AM–4:30 PM OR 2:00 PM–10:00 PM

We are seeking an experienced Care Navigator to support patients and families within a fast-paced Emergency Department and Inpatient healthcare environment.

This position is ideal for someone who is compassionate, confident, and comfortable independently engaging with patients and families while collaborating closely with nurses, social workers, and other members of the clinical care team.

Responsibilities
  • Engage directly with patients and families in Emergency Department and Inpatient settings
  • Independently introduce yourself to patients, identify needs, and provide appropriate support
  • Connect patients and families with available healthcare, social, and community resources
  • Collaborate with nurses, social workers, and other clinical team members to coordinate services

Requirements
  • 2–5 years of relevant professional experience
  • Previous Emergency Department and/or Inpatient experience strongly preferred
  • Experience working in a clinical healthcare environment
  • Comfortable independently approaching and communicating with patients and families
  • Strong verbal communication and interpersonal skills
  • Ability to effectively collaborate with nurses, social workers, and other healthcare professionals
  • Strong patient service and customer service skills
  • Ability to identify patient needs and connect individuals with appropriate resources
  • Strong organization, prioritization, documentation, and follow-through skills
  • Ability to work independently while functioning effectively as part of a multidisciplinary team
  • Professional, compassionate, and culturally sensitive approach to patient interactions
  • Ability to maintain patient confidentiality and follow healthcare policies and procedures
  • Must be available to work Saturdays and Sundays from 9:30 AM–6:00 PM
Education
  • High school diploma or equivalent required
Preferred Qualifications
  • Bachelor’s degree in Social Work, Psychology, Public Health, Human Services, Sociology, Gerontology, or a related field
  • Bilingual and/or bicultural skills reflective of the communities served
