We are seeking an experienced, compassionate, and highly professional Care Navigator to join our team. This role is ideal for someone who is passionate about supporting older adults and vulnerable populations while helping individuals and families navigate complex healthcare, community, and long-term care systems.
Our Care Navigator serve as trusted advisors and advocates, helping clients maintain independence, access appropriate resources, and achieve the highest possible quality of life. This position combines client advocacy, care coordination, problem-solving, and relationship management.
We welcome candidates from a variety of professional backgrounds who have demonstrated success working with older adults or other vulnerable populations, including children, individuals with intellectual disabilities, and those requiring long-term support services.
Equivalent experience may be considered in lieu of a specific degree. For example, candidates who have held leadership positions within senior-serving organizations, memory care programs, villages, or community-based aging services may be excellent fits.

The majority of our clients are located in Montgomery County, MD and Washington, DC. Candidates located in these areas are preferred.

While much of the work can be performed remotely or from a home office, candidates must have reliable transportation and be willing to travel to client homes and community locations as needed.
