About This Role
The Care Manager, Telephonic Nurse assesses and evaluates members' needs in a telephonic environment to achieve and maintain optimal wellness. This role is ideal for experienced nurses who are skilled in care coordination and resource facilitation.

What You'll Do
- Conduct telephonic assessments to identify members' clinical, behavioral, social, and environmental needs.
- Identify and resolve barriers that hinder effective care or treatment adherence.
- Develop, implement, and update individualized care plans as appropriate.
- Monitor member progress toward desired outcomes through ongoing assessment and evaluation.
- Provide education and support for chronic conditions management and self-care.
- Coordinate care and facilitate access to appropriate providers, services, and community resources.
- Collaborate with members, families, providers, and interdisciplinary teams to support care goals.
- Maintain timely and accurate documentation in accordance with established guidelines and quality and compliance requirements.
- Apply department, segment, and organizational policies, procedures, and operational objectives in day-to-day work.
- Exercise independent judgment and utilize available resources in managing assigned caseloads.
Requirements
Education:
- Bachelor of Science in Nursing (BSN) preferred
Experience:
- 3+ years of recent acute care clinical nursing experience
- Experience providing education and support for chronic condition management
- Experience with community resource coordination
Licenses & Certifications:
- Active, unrestricted Registered Nurse (RN) license in the state of South Carolina with multistate compact privileges and no disciplinary action required
