Humana is seeking an Onsite Care Coordination Nurse to conduct face-to-face assessments of newly admitted members and lead discharge planning. This role supports patients through admission, discharge, and post-discharge processes with a focus on whole-person care.

You will collaborate with providers and care teams, educate patients on diagnoses and medications, and ensure timely documentation while maintaining infection control and privacy standards.

Qualifications

• Current, unrestricted RN license in Missouri
• Minimum of 3 years recent acute care nursing experience
• Experience with discharge planning or care coordination in a hospital or health plan setting
• Valid driver’s license and proof of personal vehicle liability insurance

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support and educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement strategies to address these issues.
• Collaborate with attending providers, facility case managers, and other healthcare professionals for coordinated care.
• Provide education on diagnoses, medications, and preventive measures to promote ongoing wellness.
• Complete all required documentation accurately and promptly.

About This Role

As an Onsite Care Coordination Nurse at Humana, you will support members during critical moments in their care journey, providing face-to-face assessments and leading care coordination efforts for newly admitted members. This role is ideal for a nurse who is passionate about whole-person care and thrives in an autonomous environment.

Highlights

• Pay: $71,100 - $97,800/year


• Location: Remote, MO


• Facility: NKC Health Hospital


• Department/Unit: Care Coordination


• Schedule: Monday through Friday, 8:00 AM – 5:00 PM local time


• Residency: Must reside within a 30-60 minute driving distance to NKC Health Hospital

What You'll Do

• Conduct comprehensive, face-to-face assessments of newly admitted members at assigned acute care facilities.


• Support and educate members and families on discharge planning from admission through post-discharge.


• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement strategies to address these issues.

Requirements

Education:

• Current, unrestricted Registered Nurse (RN) license in the state of Missouri

Experience:

• Experience with discharge planning or care coordination in a hospital or health plan settings

Licenses & Certifications:

• Current RN License in Missouri required


• Valid driver’s license and proof of personal vehicle liability insurance required
