Humana is seeking a Field Care Manager Nurse to assess and evaluate members' needs to promote optimal wellness. This role is ideal for nurses who are skilled in case management and guiding members and families toward appropriate resources.

Responsibilities include creating individualized plans of care, coordinating with internal and external stakeholders, visiting Medicaid members in various care settings, and supporting MLTSS or LTSS as needed.

Qualifications

• Education: RN in Illinois without disciplinary action.
• Education: BSN preferred.
• Experience: 2+ years in in-home case/care management.

About This Role

The Field Care Manager Nurse at Humana is responsible for assessing and evaluating members' needs to promote optimal wellness. This role is ideal for nurses who are skilled in case management and have a passion for guiding members and families toward appropriate resources.

Highlights

• Pay: $71,100 - $97,800 per year


• Location: Remote, IL


• Residency: Must reside in the state of Illinois


• Schedule: Full-time, Monday - Friday; 8am - 5pm CST


• Travel: 75-90% local travel

Requirements

Education:

• Registered Nurse in the state of Illinois without disciplinary action


• Bachelor of Science in Nursing (BSN) preferred

Experience:

• 2+ years of experience in in-home case/care management


• Experience working with the adult population

Licenses & Certifications:

• One of the following licenses required: RN (Registered Nurse) - State Licensure required


• Certification in Case Management preferred


• Motivational Interviewing Certification preferred
