Job summary

Shirley Ryan AbilityLab is seeking a Care Manager to coordinate pre-admission clinical and financial information and guide patients through the rehabilitation pathway. You will work with the clinical team to assign patients to the correct path and monitor progress toward goals.

The role requires nursing or social work credentials, strong communication, and advocacy skills, plus experience in rehabilitation care and collaboration with external case managers.

Qualifications

• Nursing care managers must have a Bachelor’s in Nursing and 2 years of experience in rehabilitation nursing.
• Social work care managers must hold an MSW.
• Illinois license or certification in one's own profession.
• Analytical ability is required.
• Interpersonal communication and advocacy skills are required.

Responsibilities

• Serves as the liaison for the clinical team for pre-admission clinical and financial information.
• Evaluates assessment information, assigns patients to the appropriate clinical path, and monitors with the physician and team.
• Disseminates pertinent family dynamics to the team.
• Coordinates patient care by facilitating team conferences and overseeing DME and supplies.
• Monitors patient and family goal attainment relative to treatment goals.
• Maintains communication with external case managers and primary care physicians for pre-authorization.
• Advocates for patients to receive appropriate rehabilitation services.
• Negotiates with third party payor representatives.
• Participates in planning and execution of educational programs for external groups.
• Performs other duties as assigned.
