Job summary

NYU Langone Hospital—Suffolk is seeking a Care Management Assistant to join our Care Management team. This full-time, days position supports discharge planning and coordinates post-hospital care under the guidance of the RN Care Manager and LCSW.

Responsibilities include setting up post-acute services, maintaining case files, updating referral information, and communicating with payors and providers to facilitate timely discharges. Strong organizational skills and teamwork are essential.

Qualifications

• High School Diploma or GED required.
• Knowledge of medical terminology and basic computer skills.

Responsibilities

• Participates in discharge planning to ensure timely patient discharge and linkage with post-acute providers.
• Assists with collecting and transmitting information for post-discharge care and referrals.
• Maintains case files, updates referrals, and verifies health insurance details as directed.
• Supports care managers or social workers in discharge planning and review of treatment progress.
