Qualifications

• Three years of relevant experience in caregiving fields.
• Attention to detail and strong organizational capabilities.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Conduct member intake and assessments for new referrals.
• Recruit and manage direct care staff as needed.
• Develop and maintain Individualized Plans of Care & assessments.
• Monitor health status and provide necessary referrals.
• Complete progress notes and documentation for care delivery.
