Job summary

Your Health Organization in South Carolina is seeking a Clinical Scheduler to support Care Manager scheduling for Aiken patients, ensuring timely appointments and coordinated care. The role is full-time and office-based, operating 8:00 AM–5:00 PM, Monday–Friday, in the Aiken, SC office.

Qualifications

• High school diploma or equivalent required, Associate's or Bachelor's in Healthcare preferred.
• Preferred CMA, LPN or RN
• Previous experience in a medical or healthcare setting, preferably scheduling or admin.
• Familiar with medical terminology, procedures, and insurance verification.
• Strong organizational skills and multitasking ability.
• Excellent communication and interpersonal skills with all levels.
• Strong problem-solving abilities and ability to work under pressure.

Responsibilities

• Register and schedule patients according to the care model.
• Track new patients and ensure they are scheduled.
• Schedule appointments considering care model, urgency, and availability.
• Coordinate schedules across disciplines to optimize coverage.
• Follow discharged patients and coordinate post-discharge appointments.
• Schedule TC M appointments promptly within required windows.
• Maintain strong documentation and use multiple scheduling systems.
• Communicate with patients via email, portal, phone, and in person to gather info and provide instructions.
