Job summary

Entyre Care is seeking a detail-oriented Care Manager to conduct home visits, train, and support caregivers, ensuring personalized care plans and high-quality home care. You will monitor client health and coordinate with physicians, aiming to meet each family’s needs with compassion.

Requirements include a Bachelor’s degree and active LSW/LISW, with travel within Ohio. The role offers a supportive startup-like culture and meaningful work in elder care.

Qualifications

• Bachelor's degree required and must be LSW or LISW in human services, social work, or related field.
• At least one year of experience with elders or adults with disabilities.
• Valid driver's license and automobile insurance; travel within Ohio is required.
• Demonstrated experience in case management, care planning, and assessment in a healthcare setting.
• Master's degree preferred; certifications in gerontology or case management preferred.

Responsibilities

• Home visits to assess and create personalized care plans for clients and caregivers.
• Train and support caregivers to meet loved ones' needs.
• Monitor and report on clients’ health; coordinate with primary care physicians and community resources.
• Facilitate smooth transitions and ongoing care for clients.
• Maintain compliance with relevant regulations and standards; participate in community events to enroll new caregivers.
