Job summary

Providence is seeking a Case Manager RN to evaluate and manage the changing health and service needs of patients within the care coordination team in Seattle, WA. You will collaborate with the patient’s PCP and the health team to develop and implement written care plans within documented time frames.

Job description

As an integral member of the CHAT or ACO team, the Case Manager RN evaluates and manages the changing health and service needs of the patient. The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members. Collaborates with the patient's Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

Required Qualification:

• Washington Registered Nurse License upon hire.

Preferred Qualifications:
