Providence Swedish Medical Center in Seattle is seeking a Case Manager RN to evaluate and manage the changing health and service needs of the patient and family. You’ll collaborate with the patient’s PCP and care team to implement a written care plan within documentation standards.

Qualifications

• Washington RN license required upon hire.
• Bachelor's Degree in Nursing preferred.
• Three years current experience in a health care environment with home health case management experience.

As an integral member of the CHAT or ACO team, the Case Manager RN evaluates and manages the changing health and service needs of the patient. The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members. Collaborates with the patient's Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

Required Qualification

• Washington Registered Nurse License upon hire.
