Providence Swedish is seeking a Case Manager RN to evaluate and manage the evolving health and service needs of patients within the CHAT/ACO teams. You will collaborate with the patient’s PCP and health team to craft and implement a written care plan within documented time frames.

Qualifications

• Washington RN license required upon hire.
• Three (3) years of current health care experience with home health case management preferred.

Responsibilities

• Evaluate and manage changing health and service needs of the patient.
• Collaborate with the Primary Care Physician and health team to create and implement a written care plan.
