Job summary

Providence Swedish in Seattle is seeking a Nurse Case Manager to coordinate care for selected patient populations across the acute care episode. You will lead collaborative efforts with interdisciplinary teams to improve patient care, resource utilization, and outcomes.

The role emphasizes leadership in achieving clinical and financial goals while maintaining high-quality, cost-conscious care in a multidisciplinary environment.

Qualifications

• Bachelor's degree in Nursing from an accredited program or higher in a health care field.
• If no BSN at hire, you must pursue and attain BSN within 3 years.
• Washington RN license required at hire.
• Minimum 3 years RN experience in an acute care hospital.

The nurse case manager coordinates the care and service of selected patient populations through the acute care episode, across the continuum. Works collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve desired clinical, financial, and resource outcomes.

Required Qualifications:

• Bachelor's Degree Graduate of an accredited Bachelors Nursing degree program or completion of an accredited Nursing program with a Bachelors degree or higher in a health care field or,
• Applicants without a BSN degree must actively pursue and attain a BSN within three years of hire. Continued employment by Swedish beyond three years is contingent upon attaining the BSN degree.
• Upon hire: Washington Registered Nurse License
• 3 years Registered nursing experience in an acute care hospital.

Preferred Qualifications:

• 1 year Hospital case management experience.
