Job summary

Trillium Health Resources is seeking a Care Manager – RCT to join our Mid State regional team. This remote position supports Tailored Care Management for members, coordinating a multidisciplinary care team and promoting integrated, person-centered care. 8:30 am–5:00 pm hours with travel as needed.

The ideal candidate will hold QP status or NC RN licensure with MH/DD/SA experience, plus relevant master’s or bachelor’s level credentials and a valid driver’s license.

Qualifications

• Must meet Qualified Professional (QP) status.
• Fully licensed by the NC governing board for Human Services professions; OR fully licensed RN licensed in NC with MH/DD/SA experience.
• Master’s degree in human services with 1 year MH/DD/SA experience.
• Bachelor’s degree in human services with 2 years MH/DD/SA experience.
• Bachelor’s degree in another field with 4 years MH/DD/SA experience.

Responsibilities

• Provide oversight, outreach, engagement, and support of Tailored Care Management.
• Facilitate comprehensive assessments of members’ needs and outcomes.
• Initiate and maintain integrated care plans across physical, mental health, social support, and lifestyle aspects.
• Facilitate regular meetings with members, face-to-face or by phone.
• Collaborate with providers to share relevant care information.
