Job summary

Qualifications

• Wisconsin Certified Social Worker certification with at least 1 year of experience in Family Care populations.
• Bachelor’s degree in Human Services or a related field with 1 year of Family Care experience.
• A four-year bachelor’s degree in any field with at least 3 years of Family Care experience.

Responsibilities

• Conduct comprehensive assessments of member outcomes, needs and risks with IDT involvement.
• Coordinate care and services based on member-centered plans and DHS requirements.
• Participate in home visits and care conferences to reassess long-term care needs.
