Job summary

Lakeland Care is seeking a Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities in Wisconsin. The role provides care management and service coordination based on comprehensive assessments, ensuring member-centered plans and DHS contract compliance.

You will collaborate with RN Care Manager to coordinate services, participate in home visits and care conferences, and support member rights throughout the appeals process while upholding HIPAA and privacy

Qualifications

• Wisconsin Social Worker licensure or related bachelor’s/advanced degree with relevant Family Care experience
• Experience with health and long-term care services preferred
• Competency with computers and data entry

Responsibilities

• Conduct comprehensive assessments of members’ outcomes, needs and risks as part of the interdisciplinary team
• Coordinate acute and primary care services, care transitions and follow-up care with RN Care Manager
• Develop and monitor member-centered plans, authorizing services and selecting providers
• Coordinate benefits and access to federal/state programs; educate on rights to appeal and grievances
• Participate in on-going training and maintain HIPAA and DHS compliance
• Create and maintain member records and participate in home visits and care conferences
• Participate in 24-hour on-call rotation and team meetings
