Job summary

Lakeland Care, Inc. in Wisconsin is seeking a Care Manager to join an interdisciplinary team (IDT) serving frail elders and adults with disabilities.

You will perform comprehensive assessments, coordinate services, and develop member-centered plans based on DHS requirements. Under the supervision of the Care Management Supervisor, you will monitor care, educate members about rights and appeals, participate in 24-hour on-call rotations, and ensure compliance with HIPAA and program policies across

Qualifications

• WI Certified Social Worker with at least 1 year experience in Family Care populations.
• Bachelor’s degree in Human Services or related field with 1 year Family Care experience.
• Bachelor’s degree in any field with 3 years Family Care experience.
• Computer literacy, typing and data entry.
• Ability to access members’ homes and multi-task in a fast-paced environment.
• Ability to lift up to 25 lbs and maintain mobility.
• Current driver’s license with acceptable driving record.

Responsibilities

• Conduct comprehensive assessment of member outcomes, needs and risks.
• Coordinate with RN Care Manager for services, transitions and follow-up care.
• Develop, monitor and evaluate member-centered plans considering cost and effectiveness.
• Coordinate care and benefits to access federal/state programs.
• Implement risk mitigation to promote health, safety and independence.
• Educate members on rights to appeal and participate in processes.
• Participate in ongoing training; ensure compliance with regulations and policies.
• Create and maintain member records per DHS contract and policy.
• Coordinate home visits and care conferences to reassess long-term needs.
• Participate in 24-hour on-call rotation and team meetings.
• Maintain confidentiality of PHI per HIPAA and state/federal laws.
