Job summary

SeaStar Senior Services LLC in Eugene is hiring a part-time care manager to coordinate in-home care for seniors. You will perform assessments, develop care plans, and liaise with families and facilities to ensure quality care.

Requires RN license or MSW degree, experience with seniors, and a valid driver's license. Compensation is $45–$50 per hour DOE; on-call stipend provided. In-person position with flexible hours and growth potential.

Qualifications

• Good observational and writing skills.
• Must have excellent listening skills with critical thinking.

Responsibilities

• Perform comprehensive assessments gathering health history, family and social dynamics, and environmental needs.
• Create care plans enlisting supports and arranging access to services.
• Implement care plan actions to reach goals established during assessment.
• Update assessments and care plans quarterly or as needed.
• Respond promptly to changes in client condition or environment.
• Determine housing needs and assist in placement for higher level of care.
• Attend care conferences and liaise between facilities, family, and fiduciaries.
• Coordinate medical appointments, follow-ups, procedures, and medications.
• Obtain other needed professional services including legal and financial care.
• Provide or arrange transportation to appointments and outings.
• Document work with notes and time; provide monthly reports.
• Participate in weekly on-call rotation for evenings and weekends.
• Perform other related duties as assigned.

SeaStar Senior Services is a professional in-home care and care management company in Portland, Oregon and is looking for a professional Care Manager to work in Eugene.

Position Details:

This position starts at 5-10 hours per week with opportunity to increase hours as our client base grows. Requires flexible availability during business hours and participation in after-hours on-call.

Duties

• Perform comprehensive assessments, gathering information about health history, family and social dynamics, natural support systems, and environmental concerns and needs.
• Create thorough care plans enlisting supports already in place and arranging access to other needed services.
• Implement care plan actions to reach goals established during assessment and care planning.
• Update assessments and care plans quarterly or as needed.
• Respond in timely manner to changes in client's condition or environment
• Determine housing needs and assist in placement for higher level of care or arranging resources for client safety to remain in their home
• Attend care conferences and serve as liaison between facilities, family, and professional fiduciaries
• Coordinate all medical appointments, specialty follow up visits, procedures, and obtain needed medications.
• Obtain other needed professional services including legal, financial, care, and more.
• Provide or arrange for transportation to appointments and social outings
• Document all work performed on client's behalf with detailed notes and accurate time or provide monthly reports to responsible parties as desired
• Participate in weekly on-call rotation for evenings and weekends.
• Perform other related duties as assigned

Qualifications

• Registered Nurse License OR Master's in Social Work degree REQUIRED
• Experience working with seniors and adults with disabilities
• Excellent driving record and own car
• CPR certified or able to obtain
• Good observational and writing skills
• Must have excellent listening skills with critical thinking
• Computer skills required

Job Types:

Part-time, Per Diem

Pay:

$45.00 - $50.00 per hour DOE, $100 a week on call stipend

Expected hours:

5 - 10 per week to start, will increase as clients onboard
