Job summary

Community Care is seeking a Care Manager Mentor to join our team in a full-time, salaried role. Based in Waukesha with telework currently being the norm, this position supports on-the-ground community care for older adults and individuals with disabilities.

The Care Manager Mentor will train new managers, coordinate progress with supervisors, and coach staff to uphold evidence-based practices while ensuring quality care and staff retention across Family Care or Family Care Partnership settings.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or Human Services; 18 months experience with older adults or disabilities in social service capacity OR a Masters’ Degree in Social Work, Psychology or Human Services.
• Masters’ Degree in Social Work, Psychology or Human Services is required on top of the bachelor’s option.
• Long-term care functional screener certification is required

Responsibilities

• Provides on-the-job training, orientation and feedback to new Care Managers related to their assigned caseloads and tasks.
• Regularly communicates with the direct supervisor information about new employees’ progress and needs for improvement and/or educational training.
• Coaches staff to achieve the highest level of evidence-based practice and retains talent.
• Responsible for all job-specific responsibilities listed in the Care Manager job description for Family Care or Family Care Partnership, as appropriate.
