Job summary

Community Care is seeking a Case Manager to join our Wisconsin based team. The role offers a hybrid weekday schedule and a generous $8,000 sign-on bonus, with responsibilities across comprehensive assessments, care planning, and coordinated service delivery.

The Case Manager will manage caseloads in collaboration with an RN Case Manager, perform functional screens, and ensure documentation compliance while supporting seniors and adults with disabilities in the community.

Qualifications

• Bachelor's Degree in a Human Services or closely related area required.
• Three (3) years’ experience with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human services degree.
• One year of experience with older adults or persons with physical and/or intellectual disabilities preferred.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Responsibilities

Case Manager Job Requirements:

• Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
