Job summary

Molina Healthcare seeks a skilled clinician to support care management and LTSS coordination. You will work with an interdisciplinary team to enroll in waivers, develop care plans, and monitor outcomes, helping members achieve optimal health and cost-effective care.

Requires 2+ years in health care with LTSS experience, a valid driver's license, and strong communication skills. Travel will be needed to meet state requirements and member needs.

Qualifications

• At least 2 years in health care; minimum 1 year in care management or LTSS
• Licensed/credentialed social work or related professional license per state requirements
• Experience with disabilities, chronic conditions, substance use, LTSS
• Must have valid driver's license and reliable transportation

Responsibilities

• Complete comprehensive member assessments within deadlines, including home visits
• Manage waiver enrollment/disenrollment processes
• Develop and implement care plans with members and care network
• Monitor care plan effectiveness and adjust as needed
• Coordinate services with interdisciplinary team for approved services
• Assess barriers to care and assist with psycho/social/financial obstacles
• Travel up to 25–40% as required by state/contractual needs
