Qualifications

• At least 2 years healthcare experience, including LTSS and care management experience.
• Active LPN/LVN license; if required by state, license must be valid.
• Valid driver's license, reliable transportation, and ability to travel as needed.
• Proficient in Microsoft Office and navigating online portals.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits when required.
• Facilitate waiver enrollment/disenrollment and develop care plans with stakeholders.
• Monitor care plan effectiveness, document interventions and adjust as needed.
• Collaborate with interdisciplinary care teams to coordinate services.
• Travel 25-40% based on state/contract requirements.
