Job summary

Molina Healthcare is seeking a skilled care coordinator to support management and coordination of long-term services and supports across the care continuum. The role focuses on ensuring member progress toward outcomes, collaborating with a multidisciplinary team, and promoting cost-effective quality care.

Responsibilities include completing assessments (including home visits), developing and monitoring care plans, coordinating waiver services, and facilitating ICT meetings.

Qualifications

• Minimum 2 years health care experience including LTSS and care management.
• Active LPN/LVN license; if required by state contract, license must be active/unrestricted.
• Valid driver’s license and reliable transportation.
• Strong communication, problem solving and time management skills.
• Proficiency with Microsoft Office and health portals/databases.

Responsibilities

• Completes comprehensive member assessments within required timelines, including home visits.
• Facilitates waiver enrollment/disenrollment processes and develops care plans with stakeholders.
• Monitors care plans, evaluates effectiveness and suggests changes.
• Promotes integrated services across behavioral health and LTSS.
• Assesses medical necessity and coordinates appropriate waiver services.
• Stewards interdisciplinary care team meetings for service approvals.
• Educates and motivates members using motivational interviewing and clinical guidelines.
• Identifies barriers and coordinates solutions for psycho/social, financial, and medical obstacles.
