Job summary

Molina Healthcare is seeking a care manager to support LTSS-focused activities and coordinate member care across the continuum. You will work with a multidisciplinary team to ensure progress toward outcomes and quality, cost-effective care. Expect 25-40% local travel as required by state contracts.

The role emphasizes care planning, monitoring, and collaboration with caregivers, physicians, and licensed professionals to address psycho/social, financial, and medical barriers to care.

Qualifications

• At least 2 years of health care experience, including 1 year with persons with disabilities or LTSS, and 1 year in care management.
• Licensure and/or certification required ONLY if mandated by state contract/regulation; active and unrestricted if licensed.
• Some states may require a bachelor’s degree in a health care-related field.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job travel.
• Demonstrated knowledge of community resources.
• Ability to work across diverse settings and populations with minimal supervision and self-motivation.
• Strong written and verbal communication; proactive, detail-oriented, and able to adapt to change.
• In some states, at least one year of experience with individuals with substance use disorders.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as needed.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and others.
• Monitor care plans, document interventions, and adjust as needed.
• Promote service integration across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize appropriate waiver services.
• Evaluate benefits and advise on funding sources.
• Facilitate interdisciplinary care team meetings for approvals and collaboration.
• Use motivational interviewing to educate and motivate members.
