Job summary

Molina Healthcare, Inc. is seeking a care management professional to support LTSS activities and coordinate member care across the continuum. You will engage in comprehensive assessments, develop and monitor care plans, and work with interdisciplinary teams to ensure high-quality, cost-effective member care.

The role requires travel (25-40%), strong communication, and experience with waiver services. A healthcare background and state-regulated licensure or eligibility are preferred.

Qualifications

• 2+ years health care experience, LTSS exposure, and care management experience.

Responsibilities

• Complete comprehensive member assessments within timelines and visit requirements.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, and health professionals.
• Monitor care plans for effectiveness and adjust as needed.
• Promote integrated LTSS and behavioral health services across the care continuum.
• Assess medical necessity and authorize waiver services.
• Support ICT meetings for service approvals or denials.
• Educate and motivate members using motivational interviewing techniques.
• Identify barriers to care and coordinate solutions.
• Coordinate with licensed care managers as needed.
• Travel up to 40% depending on state requirements.
