Job summary

Molina Healthcare is seeking a skilled Care Manager to support long-term services and supports (LTSS) across the continuum. You will collaborate with a multidisciplinary team to coordinate member care, ensuring progress toward outcomes and alignment with quality and cost goals.

The role requires experience in care management, strong communication, and the ability to travel 25-40% based on state requirements.

Qualifications

• Licensed clinical social worker or equivalent with active license in state of practice.
• Minimum 2 years in health care with care management/LTSS exposure.
• Experience working with persons with disabilities or chronic conditions preferred.
• Must have valid driver's license and ability to travel 25-40% as required.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits as needed.
• Coordinate waiver enrollment/disenrollment processes and develop care plans with members and care network.
• Monitor care plan effectiveness and adjust interventions to achieve goals.
• Promote integration of services across behavioral health and LTSS.
• Identify barriers to care and provide coordination to address psycho/social and financial obstacles.
