Qualifications

• Minimum 2 years healthcare experience with LTSS or related field.
• Licensure/certification required only if state contract mandates.
• Valid driver license and reliable transportation for travel.

Responsibilities

• Complete comprehensive member assessments within timelines and in-home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document outcomes.
• Coordinate LTSS services across disciplines to ensure continuity of care.
• Identify barriers to care and provide navigation support.
