Community Health Systems is seeking a Licensed Social Worker-Care Manager at Longview Regional Medical Center in Longview, TX to coordinate discharge planning and transitions of care for patients.

Qualifications

• Bachelor's Degree in Social Work required.
• Master's Degree in Social Work preferred.
• 2-4 years of experience in an acute care hospital setting required.

Responsabilités

• Collaborates with interdisciplinary teams to coordinate patient care and discharge planning.
• Develops discharge plans and coordinates post-hospital placement and social services.
• Refers cases to physicians or managers when criteria are not met; ensures timely interventions.
• Maintains accurate documentation of case management activities and compliance with reporting requirements.
