Job summary

Episcopal Retirement Services is seeking a Care Manager for our Living Well Senior Solutions in Cincinnati, OH. You will assess client needs, develop care plans, and coordinate services with families, providers, and referrals to support aging in place.

The role emphasizes client autonomy, advocacy, and collaboration with ERS teams to ensure high-quality care and satisfaction while meeting regulatory standards and maintaining flexible hours.

Qualifications

  • Bachelor’s degree in social work, nursing, or health care related field with a current Ohio license, if applicable
  • Experience in care management and/or in-home services preferred
  • Willing to become ALCA certified if required
  • Possesses a valid driver’s license and reliable transportation
  • Extensive knowledge of older adult needs and available programs
  • Willing to work flexible hours per client needs
  • Strong rapport with agency referral sources
  • Proficient in Microsoft Office and Outlook; willing to learn documentation system

Responsibilities

  • Provide unbiased, comprehensive assessment to determine client strengths and challenges
  • Set goals and coordinates information and options for care decisions
  • Develop flexible care plans and monitor client needs
  • Assist with determining level of care and transfers when needed
  • Coordinate with services and arrange referrals as requested

Position Purpose: The purpose of the Aging Life Care™ Manager is to assist clients and their families/friends as they navigate through the complex issues associated with aging. This holistic, client-centered approach includes assessing for strengths and challenges, providing guidance, development of solutions and coordination of services. The Care Manager will allow maximum decision making to honor the autonomy of the older adult receiving the care, in accordance with current federal, state and local standards, guidelines, personal licensure and regulations, and ensure that all services are in accordance with ERS’ standards and at or above ERS’ expected levels of client satisfaction.

Minimum Qualifications:

  • Bachelor’s degree in social work, nursing, or health care related field with a current Ohio license, if applicable
  • Experience in care management and/or in-home services preferred.
  • Willingness to become certified through ALCA if required.
  • Possesses a valid driver’s license, reliable transportation, and appropriate insurance coverage.
  • Extensive knowledge of older adult needs and support and programs available.
  • Willing to work very flexible hours including being available according to client needs.
  • Ability to develop an excellent rapport with agency referral sources.
  • Strong computer skills in Microsoft Office and Outlook. Willing to learn documentation system.
  • Able to work independently with minimal supervision and to handle multiple tasks while meeting deadlines.
  • Able to read, write, and understand the English language and speak the language in an understandable manner.
  • Possess sight/hearing sense or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must be able to stand, sit, bend, stoop, lift, and push up to 25 pounds on a regular basis.
  • Must be fully up to date with required vaccinations.

As a Care Manager you can expect to:

  • Provides unbiased, comprehensive assessment to determine client strengths and challenges.
  • Determines goals, priorities and assures desired results align with client priorities. Offers possible solutions, provides resources, and coordinates information and options regarding care decisions.
  • Develops a flexible plan of care in conjunction with the client and service providers. Monitors and manages the care plan as client needs change.
  • Assists with determining level of care needed, next steps and transfers, if required, to appropriate level of care in a facility.
  • Screens, arranges and partners with other services as needed/requested.
  • Assists the client’s local and distant family members and care partners to facilitate quality, coordinated care and expectations. Acts as a liaison.
  • Reviews financial, legal, or medical issues and offers referrals to specialists as needed.
  • Acts as an advocate for clients, recognizing the clients’ values rather than their own. Demonstrates sensitivity to client and/or family/care partner needs, preferences, customs and feelings.
  • To the greatest extent possible, fosters self-determination on the part of the older adult to facilitate living in accordance with his/her personal values and goals.
  • Effectively communicates with all involved based on confidentially criteria.
  • Ensures that all services provided to clients are in accordance with ERS’ standards and meet ERS’ expected levels of client satisfaction.
  • Accompanies client to medical appointments and procedures when appropriate.
  • Maintains regular in-person, phone, and/or written contact with clients, families, staff, referral sources, client’s physicians/specialists and outside community health, welfare and social agencies and others as needed to provide effective services to coordinate services and monitor client status.
  • Counsels, coaches, and educates the client, family, and care partners as needed to ensure the client’s needs are met.
  • Provides guidance, intervention, and advocacy during a crisis.
  • Assures community clients are prepared in the event of an emergency.
  • Interacts, supports, and develops plans with ERS teams and various departments (Health Services, Meals on Wheels, Center for Memory Support and Inclusion, Middle Market, Fund Development, Wellness, etc.) to promote and integrate services and resources.
