Providence Medical Foundation seeks a licensed Clinical Social Worker to join our care management team in Anaheim, CA. You will coordinate services, support patients and families, and collaborate with physicians, nurses, and other care team members to address psychosocial needs.

The role emphasizes compassionate, patient-centered care, strong communication, and adherence to state licensing requirements. Travel may be necessary, and driving may be required as part of patient visits.

• California Clinical Social Worker License required at hire.

Care Management is a collaborative practice model including the patients, nurses, Care Manager LCSWs, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient.

Required Qualifications:

• Master's degree in social work
• Upon hire: California Clinical Social Worker License
• Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
• 2 years of Relevant Social Work experience in a medical group, health plan, hospital, community, post-acute, or medical office/clinic setting.

Preferred Qualifications:

• 3 years of Care management and Managed Care experience in acute, ambulatory, or community health setting.
• 3 years of Behavioral Health experience in acute or outpatient setting.
