Qualifications

• BSN degree required or demonstrated success in CHRISTUS RN Care Manager I position.
• Master’s degree in nursing preferred.
• Program management experience managing multiple projects preferred.
• Excellent verbal and written communication.
• Critical and analytical thinking skills.
• Demonstrated clinical competency.
• Working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
• Understanding of pre-acute and post-acute levels of care and community resources.
• Ability to work independently and exercise sound judgment in interactions with physicians, payors, patients, and families.

Responsibilities

• Coordinate and facilitate patient care progression across the continuum.
• Implement and monitor the plan of care for effectiveness.
• Identify and escalate system barriers impeding progress.
• Proactively resolve delays to discharge.
• Collaborate with medical staff to remove barriers to care.
• Ensure plan of care is communicated to patients, families, and team.
• Manage all aspects of discharge planning for assigned patients.
• Provide education and support related to goals of care.
• Advocate for patient and coordinate care to the appropriate next level.
