Job summary

Mental Health Cooperative in Antioch, TN is seeking a Care Manager I for children and youth with SED and their families, and adults with SPMI, to help them achieve healthier, more independent lives in the community.

You will collaborate with an interdisciplinary team, conduct community visits, and develop person-centered care plans, with a Monday–Friday schedule and a base salary around $41,000 plus language/behavioral health incentives.

Qualifications

• Bachelor’s Degree in a health-related field required.
• If degree is in criminal justice, 15 college hours in behavioral health required.
• Valid Tennessee Driver’s License and clean MVR.
• Acceptable Criminal Background Investigation and personal auto insurance.

Responsibilities

• Provide quality consumer care and engage with consumers monthly to assess needs and care plans.
• Complete community visits (home, school, court) to evaluate needs and connect with individuals.
• Address mental/physical health, social determinants, gaps in care, and ED/IP utilization.
• Assist with referrals and linkages to community resources; advocate for consumers.
• Collaborate with consumers’ support system; identify/increase supports as needed.
• Follow policies to ensure care is delivered with quality and timely documentation.
• Meet productivity standards as outlined in policies.
