Mental Health Cooperative in Dickson, TN, seeks a Care Manager I, Child & Youth, to provide comprehensive care management to children and youths with SED and families, or adults with SPMI. You will develop relationships, assess needs, and help clients achieve healthy community living through a collaborative team approach.

Responsibilities include engaging with consumers in the community, coordinating care, and referencing agency policies for documentation and productivity.

Qualifikationen

• Bachelor’s Degree in a health-related field of counseling, psychology, social work, or other behavioral health field required.
• Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.
• Valid Tennessee Driver’s License
• Acceptable Motor Vehicle Report (MVR)
• Acceptable Criminal Background Investigation
• Personal Automobile Insurance

Aufgaben

• Engage with consumers monthly to assess needs and coordinate care.
• Complete visits with consumers in the community (home, school, court, etc.).
• Address mental and physical health needs, social determinants of health, and gaps in care.
• Assist with referral and linkage to community resources; advocate for consumers.
• Collaborate with consumers’ support system and increase supports when needed.
• Document timely and comprehensive care per agency policies.
• Meet productivity standards as indicated in policies and procedures.
• Participate in daily team meetings and monthly supervision; share insights and plan actions.
