The Community Council of Greater Dallas seeks an experienced case manager to support older adults 60+ and their caregivers. You will assess client needs, develop care plans, and connect clients with public benefits and community resources in Dallas County.

Most duties require independent work within agency policies, confidentiality, and timely documentation in the client tracking system. Bachelor’s degree in social work or related experience is required, with occasional evening/weekend

Qualifications

• Bachelor's degree in social work or related field required.
• One-on-one counseling experience by phone or in person.
• Experience with benefits counseling or public programs a plus.

Responsabilités

• Conduct in-person needs assessments for caregivers and older adults.
• Document client needs and develop client care plans.
• Coordinate services and share care plans with caregivers.
• Maintain client confidentiality and records.
• Authorize services from vendors after assessment and documentation.
• Submit invoices and requisitions in a timely manner.
• Represent DAAA and CCGD at public events as directed.
• Attend staff meetings, trainings, and workshops for professional development.
• Assist with intake calls and eligibility information.
