Crossroads Treatment Centers in Philadelphia is hiring a Care Manager to connect with patients monthly, address needs, and link them to community resources to support recovery.

You will collaborate with a dedicated team across outpatient clinics, travel to primary and secondary locations, and work 37–40 hours per week with alternating weekends, delivering compassionate, evidence-based care.

Qualifications

• High School Diploma or GED required.
• Experience in healthcare, addiction medicine, community health, care coordination preferred.
• Experience with building individualized care plans is a plus.
• Knowledge of compliant documentation practices.

Responsabilités

• Connect with patients monthly to address needs and provide resources.
• Build and maintain relationships with community partners to support patients.
• Work closely with a dedicated team to deliver exceptional patient care.
• Participate in community awareness/events to promote Crossroads and our approach to addiction treatment.
