Epiphany Family Services is seeking a Care Manager Extender to assist the Tailored Care Management team in Gaston County. The role focuses on outreach, enrollment, and coordination of services for members in a hybrid in-office, field, and remote setting.

You will engage with members, coordinate appointments, and support the care manager by sharing insights and assisting with resource referrals and follow-ups. A peer support background and NC CHW training are preferred.

Qualifications

• Must be 18 years of age or older.
• HS diploma or equivalent required.
• Certified Peer Support Specialist or equivalent CHW background required.

Responsibilities

• Perform outreach, engagement, and follow-up with members.
• Coordinate services/appointments and reminders, arrange transportation.
• Conduct introductory engagement calls to enroll members in TCM services.
• Share information with care team about member circumstances.
• Provide and track referrals and assist in accessing community resources.
• Support the care manager in assessing unmet health-related needs and follow up on urgent TCM needs.

TCM Care Manager ExtenderDepartment: Tailored Care ManagementLocation: Hybrid (In-Office, Field, and Remote)Service Area: Gaston CountySchedule: Monday – Friday | 8:30 AM – 5:00 PMResponsibilitiesPerforming general outreach, engagement, and follow-up with members.Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation).Conducting introductory engagement calls to enroll members into TCM services.Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing.Sharing information with the care manager and other members of the care team on the member’s circumstances.Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.Support the care manager in assessing and addressing unmet health-related resource needs to include following up on ADT alerts and other urgent TCM needs.Other duties as assigned.Qualifications18 years of age.Has a high school diploma or equivalent.1. Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist2.
