Job summary

Epiphany Family Services in North Carolina is seeking a Care Manager Extender to support Tailored Care Management by performing outreach, enrollment calls, and care coordination with members.

Hybrid work model (in-office, field, and remote) and a schedule of Monday–Friday, 8:30 AM–5:00 PM, with emphasis on timely follow-ups and resource connections.

Applicants must be 18+, have a HS diploma or equivalent, and either be a Certified Peer Support Specialist or a NC CHW with related paid experience.

Qualifications

• 18 years of age.
• High school diploma or equivalent.
• Lived experience with a behavioral health condition and a Certified Peer Support Specialist.
• One year of paid experience performing Extender functions.

Responsibilities

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments and arrange transportation.
• Conduct introductory engagement calls to enroll members into TCM services.
• Engage in health promotion activities and knowledge sharing.
• Share information with the care team about member circumstances.
• Provide and track referrals to community resources and social services.
• Support the care manager in assessing unmet health-related resource needs and follow-up on urgent TCM needs.
