Monarch Communities is seeking a Care Manager (CNA/CHHA) for Part-Time and Per Diem shifts in Livingston, NJ. The role involves assisting residents with daily living activities and following the care plan, with a weekend schedule option and afternoon/evening coverage.

The position reports to the Health & Wellness Director. CNA/CHHA credentials are required, and the role includes flexible scheduling and on-the-job training as part of a compassionate, resident-centered team.

Qualifications

• Nurse aide certification or equivalent required.
• Experience with elderly population preferred.
• Ability to document care in resident records.

Responsibilities

• Assist residents with bathing, dressing, toileting, and feeding per care plan.
• Maintain safety, dignity, and confidentiality of residents.
• Escort residents to meals and activities.
• Ensure residents are clean, dressed, safe, and fed during shifts.
• Document all provided services during the shift.
• Report changes in resident condition to supervisor.
