Job summary

Arosa in Reno, NV seeks a Care Manager to partner with Operations to coordinate and manage service delivery for clients, promoting independence and well-being.

You will assess client status, draft and revise the Plan of Care, and collaborate with families and care teams to ensure effective care coordination, documentation, and communication. Travel and on-call duties may be required; licensure and CCM/CMC certification are preferred.

Qualifications

• Bachelor’s or Associate degree in a healthcare-related field.
• Two years’ care management experience preferred; aging population experience is a plus.
• Licensure in candidate's field highly desired.

Responsibilities

• Assess and reassess client status (physical, mental health, social resources, safety, economics, functional ability).
• Draft, implement and revise the Plan of Care for appropriateness and effectiveness.
• Collaborate with clients, families, and care teams to facilitate coordination and feedback.
• Maintain documentation and ensure effective communication for continuity of care.
• Provide supportive communication and advocate for client well-being.
