Job summary

Arosa in Reno, NV seeks a Care Manager to partner with Operations to coordinate and manage service delivery for clients, promoting independence and well-being.

You will assess client status, draft and revise the Plan of Care, and collaborate with families and care teams to ensure effective care coordination, documentation, and communication. Travel and on-call duties may be required; licensure and CCM/CMC certification are preferred.

Qualifications

  • Bachelor’s or Associate degree in a healthcare-related field.
  • Two years’ care management experience preferred; aging population experience is a plus.
  • Licensure in candidate's field highly desired.

Job description

In alignment with company mission, vision and values, the Care Manager partners with Operations to coordinate and manage service delivery to clients, optimizing their functioning, independence, and overall well-being.

  • Uses care management skills to independently assess and reassess client status, including physical and mental health, social resources, environmental safety, economic resources, and functional ability.
  • Drafts, implements and revises the client Plan of Care, to ensure appropriateness, effectiveness, efficiency, and client satisfaction.
  • Collaborates with clients, families, and support teams to obtain information, provide feedback, and facilitate healthcare coordination.
  • Maintains required documentation and utilizes effective communication to facilitate continuity of care.
  • Provides supportive communication to and conflict resolution with clients and family members; serves as an advocate for the well-being of the client.
  • Makes referrals to appropriate community agencies such as crisis intervention, legal advocacy, and counseling agencies for medical, mental, physical, financial, legal, and social needs.
  • Communicates elevated care issues with appropriate internal professionals, including Area Director and Executive Team.
  • Extends client length of service through the delivery of exceptional care management services and consistently exceeding client expectations.
  • Works closely with the teammate(s) who interviews, matches, trains, and supervises in-home teammates to ensure client needs are met, when client requires caregiving services. May be required to assist in similar tasks depending upon client situation.
  • Performs all responsibilities in accordance with professional codes of ethics, Arosa policies and procedures and applicable state and federal regulations.
  • Maintains productivity target of at least 15 billable hours per week (some offices may have a higher target) based on company guidelines set for weekly care management revenue goals.
  • Serves as Care Manager on call, receiving inquiries and managing client needs evenings and weekends during on call rotation.
  • Documents and updates activity in multiple systems to ensure compliance with company protocols.
  • Attend, and as requested, provide education to others during weekly peer supervision and monthly Care Management meetings.
  • Collaborates with community organizations to promote awareness and understanding of elder care issues and Arosa services, including but not limited to, presentations, in-person meetings, and electronic outreach.
  • Participates in Arosa and office committees, programs, initiatives, and meetings as needed/requested
  • Other duties as assigned.
Supervisory Responsibilities
  • Supervise in-home and/or care management teammates, including safety monitoring, performance management, training and all other supervision.
Knowledge and Skill Requirements:
  • Educational Minimum Requirement: Bachelor’s or an Associate Degree in Social Work, Nursing, Gerontology, Psychology, Health and Human Services or other healthcare related fields. Preferred candidate: Master’s Degree in a healthcare related field.
  • Licensure in a candidate's professional field highly desired.
  • A minimum of two (2) years’ experience in care management preferred; experience working with the aging population preferred; other professional experience may substitute.
  • Attainment of a Care Management Certification (CCM or CMC) desired, when eligible (typically within the first one to two years of employment.)
  • Ability to interface effectively and professionally with client support team, including family, physicians, attorneys, etc.
  • Ability to work in a team environment required.
  • Excellent written and verbal communication skills required.
  • Valid driver’s license and proof of current auto insurance required.
