The West Philadelphia Community Mental Health Consortium, Inc. (Consortium) is a nonprofit (501(c)(3) Community Mental Health Center (CMHC) established in 1967 as one of Philadelphia’s twelve CMHCs. Throughout our history, we have pioneered providing behavioral health services with ‘compassion first’ priorities coupled with excellence in mental health treatments and collaborative understanding. For over five decades, we have helped more than 250,000 people throughout the tri-state area overcome the challenges of mental illness and substance abuse.
Located in Southwest and West Philadelphia, the Consortium is the premier provider of Supports Coordination, Outpatient Mental Health, Addictions, Case Management, Eco-systemic Family Therapy (ESFT) and Functional Family Therapy (FFT) for all ages in West and Southwest Philadelphia, serving approximately 4,000 individuals in 2022.
Summary
This position will provide support, case coordination, crisis intervention and social/recreational services in a residential setting and will assist as the back-up to the Site Supervisor on various occasions.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Completes all service plans with individuals input and psychosocial evaluations within seven days of entry;
- Completes progress notes before the end of shift, assist with case conferences on an annual basis and other documentation in compliance with Agency, County, State and Federal requirements.
- Evaluates and monitors individuals skills and level of functioning every 60 days.
- Completes an initial service plans in 30 days and thereafter every 60 days or as needed.
- Provides one-on-one assistance and coaching on daily living skills (ADL) as outlined in the service plan or as directed by supervisor.
- Performs a daily medication count. Prepares monthly medication logs, troubleshoots medication problems with supervisor, doctors, Intensive Case Managers and residents. Responsible for medication management oversight on a daily basis.
- Implements and documents medication changes when ordered by the doctor. Faxes prescription and places orders for medication refills as needed to ensure that prescribed medication is available for individuals at all times.
- Helps individuals access and maintain necessary social service support by accompanying them to the agencies, hospitals, doctors appointments, banking needs, food shopping and all required involuntary commitment hearings as needed.
- Actively participates as part of the residents treatment team as needed.
- Assists residents in acquiring alternative living arrangements prior to graduating from the program.
- Plans and participates in social and recreational activities with residents as needed.
- Assists with crisis situations which may require staying on the site or with the individual until the situation is resolved.
- Reviews the department communication log at the start of each shift and follows all noted directives on a daily basis.
- Reports all incidents to supervisor with 24 hours of the incident and completes all documentation associated with the incident i.e. communication log, incident report, progress notes, etc.
- Answers the telephone promptly and courteously. Ensures that all messages are written down and delivered to the resident(s) and other staff within a timely fashion.
- Establishes and maintains effective communication with other programs and agencies involved with the residents treatment and rehabilitation as needed.
