Position Details

This remote position involves 25% travel within Cook County, Illinois, supporting our YouthCare/Foster Care program. Applicants must reside in specified Illinois ZIP codes.

Role Responsibilities
  1. Evaluate member needs, barriers to care, and social determinants of health, focusing on member priorities.
  2. Develop care plans and collaborate with providers and community resources.
  3. Identify and address problems or barriers to care.
  4. Coordinate care among members, families, caregivers, and providers.
  5. Monitor and revise care plans based on member progress.
  6. Support members with local resource information.
  7. Facilitate timely access to care and services.
  8. Conduct outreach via phone, digital means, or home visits as needed.
  9. Document all activities in compliance with regulations.
  10. Educate members and families about health conditions and care options.
  11. Provide feedback for quality improvement.
Qualifications
  • Degree from an accredited nursing school or a Bachelor's in Nursing.
  • 2-4 years of related experience.
  • RN license in Illinois or a compact state.
Additional Information

Salary range: $55,100 - $99,000 per year. Benefits include health insurance, 401K, stock plans, tuition reimbursement, paid time off, and flexible work arrangements.
